    Birthday and Anniversay Greetings
    The Mayor gladly provides greetings to Port Coquitlam residents celebrating birthdays of 75 years and up, as well as to Port Coquitlam couples celebrating anniversaries, starting at the 40th.  Greetings are issued in five year increments, e.g 75, 80, 85 or 40, 45, 50.

    Please give us as much notice possible and include the following information:
    • Name of celebrant(s), including title (Mr., Mrs., Ms., Dr. etc.) 
    • Address of celebrant(s) 
    • Name and address of who and where to send greetings (if different from above) 
    • Date of birthday/anniversary 
    • Celebration date (if different from above) 
    • Name and phone number and/or email address of person requesting greetings

    You can provide this information by completing and submitting the request form below or by writing or faxing:

    Request a Greeting

    To receive a greeting from other government agencies, you must fill out a Congratulatory Message Request form and return it to the Protocol and Events Branch of the Provincial Government. The form is available electronically:

    • Onlinefill-in version
    • PDF(91 KB - Adobe Acrobat required) format that can be printed, filled-in manually, and then mailed or faxed.

    * Please note that nine weeks advance notice is required for processing to ensure you receive the message on time.
