MVA & Fire Incident Report
Information regarding a fire, motor vehicle accident, or other emergency incident to which the City of Port Coquitlam Fire Service responded are available upon request for a fee. Please complete the Incident Request Form at the bottom of this page.

Processing Your Request
In the event we need to clarify information, please include a contact telephone number and email address. In order to reduce processing time and additional fees, please ensure your request is clear, concise and focused and that all required fields are completed on the Incident Request Form.
Please allow 30 business days for processing.
Fees & Payment
Requests will not be completed until full payment has been received. Fees for locating, retrieving, producing and preparing records:
- Motor Vehicle Accident/Incident Report – $110.00 per incident
- Fire Incident Investigation Reports and Photos (if applicable) – $180.00 per incident
Payment can be received in the following ways:
- Credit card – in person
- Debit card – In person
- Cheque payable to “City of Port Coquitlam” – Mail or drop off at the following address:

To protect your privacy and ensure confidentiality, please do not include any personal information such as your social insurance number, personal health number, or date of birth. We do not require this information and your request may be returned to you to amend prior to being accepted.
If you require additional information that the City or Port Coquitlam Fire Service does not routinely provide, please inquire through the City’s Freedom of Information request process.
